QUESTIONNAIRE

This Questionnaire is designed to let us get to know you better and to allow us to match you with a puppy.

1. How many people are in your household?

2. Do you have any children? If yes, what are their ages?

3. If you have children have they been exposed to dogs in the past?

4. Has anyone living in your household ever had a problem with dogs?

5. Does anyone in your household suffer from allergies, asthma or have any other problem which might be aggravated by dog hair or dander?

6. Do you own or rent your home?

7. If you rent, will your landlord be willing to sign a statement allowing you to keep a dog in the apartment/house?

8. Do you have a fenced in yard?

9. Do you work full time?

10. Are there any family members at home full time?

11. How much time do you plan to spend with the puppy?

12. What are you looking for in a puppy? i.e. companion, show, obedience

13. Are you interested in a puppy of a particular sex? Are you seeking a puppy of a particular colour?

14. Is this puppy intended for breeding? If not, do you intend to spay/neuter?

15. Do you plan to do any obedience work? At what age?

16. What are your plans for exercising the puppy?

17. Are you aware of the socialization needs of a puppy and do you feel you are capable of providing for these needs?

18. Are you aware of some of the health problems which appear in the Tibetan Spaniel today?

19. Are you willing to title the puppy in obedience, therapy or conformation?

20. If you live out of province are you willing to travel to Ontario to meet the sire and dam?

21. If travel is impossible, would you be willing to allow a home visit from one of our friends in your area?

22. Are you a member of any breed clubs - regional, national or international?

23. Have you ever held office for any of these clubs? If so, was this office voluntary or elected?

24. Have you ever crate-trained a puppy?

25. What are your feelings on crate-training?

26. Do you realize how much upkeep of a Tibetan Spaniel puppy can cost?

27. Why do you want a Tibetan Spaniel?

28. How much experience have you had with this breed?

29. What are your impressions of Tibetan Spaniels you have met in the past?

30. Do you have any other pets? What pets have you had in the past?

31. Please provide the name, address and telephone number of your veterinarian as reference. If you don't have a veterinarian, have you made plans to acquire one?

32. Please provide at least two references (may include veterinarian in #31).

33. What is your opinion as to the place of a pet in the family structure?
